Appointments are NOT required for URGENT medical care. Appointments ARE required for workman's compensation visits and motor vehicle accident cases. Please call in advance to schedule an appointment.

What We Need From You
For faster service please print and fill out your patient registration forms before coming to your appointment.
Please be prepared to provide the following to our staff:
- Driver's License or any other state issue ID
- Insurance cards
- Insurance subscriber information (this includes your parent's or spouse's date of birth and Social Security number)
- If receiving treatment for an auto accident please provide the claim number issued by your auto insurance
- If receiving treatment for Worker's Compensation please bring written authorization and a working telephone number where your supervisor can be reached
Cancellations
If you cannot make it to your scheduled appointment, we ask that you give us at least 24 hours notice so that we can schedule other patients for that time slot.
